Please complete and print, 2012 Players / Cheer Application Package Liability Release Waiver TVYFL Medical Form Copy of Insurance Card Birth Certificate (New Players)

2012 PAYMENTS We understand hardships and are willing to work with any parent/guardian for a plan to pay registration fees. However, arrangements must be made ahead of time and the payment schedule must be kept. Fees must be completely paid by the beginning of the season or your child will lose his/her spot on the team.

COACHES/DADS With PADS STILL NEEDED
If you are interested in being a Coach or Dads with Pads for the 2012 Football season, you must fill out the application and return it to a board member ASAP, Coaches / Dads with Pads meetings will start in March (Dates TBD).

All coaches will will need the following: (before being approved to coach by the TVYFL)
DOJ Background check Copy of your current Driver’s License and your car insurance Copy of your CPR/First Aid certification good through November 2012 Copy of your USA Football Certified Card (Online Certification dates TBD) All coaches MUST attend the coaching clinic in July.
